The Health Information Management Department at Samaritan Hospital maintains and retrieves patients' medical records. Patient Privacy is a priority at Samaritan Hospital and we take pride in protecting your medical records.

Guidelines for Medical Records Requests

To receive medical records you must fill out an Authorization for the Release of Health Information.  You must present a photo identification before we will release a copy of your records.  If the patient is a minor (less than 18 years of age), a parent or legal guardian must sign the authorization and present a photo identification.
