Social Services

The Social Services Department is available to provide information and assistance with any hospitalization needs, financial, post-hospital care or nursing home placement. This department works closely with home health agencies, nutrition sites, in-home agencies, area nursing home and residential care facilities and the Department of Health and Senior Services.  Social Services can assist in obtaining medical equipment such as wheelchairs, walkers, and oxygen.

Social Services has information and can help complete forms regarding Health Care Directives and Durable Power of Attorney for Health Care Decisions.
